Position Summary

Coordinate and provide primary care services to patients with HIV disease, in collaboration with the center’s medical staff. Maintain individualized documentation/care plan for HIV patients based on ongoing assessment and identify services needed. Provide ongoing patient education to empower patients to take an active role in managing their health. Work closely with the broader HIV team to assure that patients receive needed services and referrals. Assist with HIV testing activities in the clinic. Offer spiritual care to patients in need.

Essential Functions

  • Coordinates services to HIV infected patients
  • Maintains individualized documentation/care plan for HIV patients and identifies services needed at upcoming visit; regularly reviews plan and confers with the patient to assure the patient is receiving effective and appropriate care.
  • Provides direct primary care services to patients with HIV disease in collaboration with the center’s medical staff.
  • Conducts intake and assessment interviews with HIV patients, and develops individualized comprehensive care plans.
  • Locates and coordinates resources and makes referrals and/or arrangements for treatment and support services.  Works in coordination with HIV case managers and social workers.
  • Provides ongoing patient education, counseling, and advocacy in coordination with the HIV care team to ensure consistent, appropriate care.
  • Works closely with the Outreach Worker to assure linkage to primary care and appropriate support services.
  • Works closely with the adherence coordinator to enhance patient adherence to HIV treatment and care.
  • Works closely with other members of the HIV nursing team including HIV Care & QI Coordinator and HIV Medical Assistant and provides assistance and coverage as needed.
  • Participates as a member of the larger HIV/AIDS care community by taking advantage of opportunities for training, networking, and professional support.
  • Performs data entry into the CAREWare system as needed.
  • Performs OraQuick Rapid HIV testing on clients at Esperanza Health Center. Maintains Universal Precautions while performing testing and uses Personal Protective Equipment per protocol. Observe all OSHA regulations with regards to the disposal of biohazard waste. Notifies HIV Testing Manager or Lab supervisor immediately if testing or control results are outside of control range or there is any variance from expected results.
  • Provides Pre-Test and Post-Test counseling to patients receiving HIV testing and documents all care provided in accordance with Esperanza Health Center OraQuick Guidelines.

Knowledge, Skills, and Qualifications

  • Licensed Registered Nurse.
  • Must be fully bilingual in English and Spanish.
  • Demonstrated understanding of HIV/AIDS-related issues and standards of care; experience in working with the population(s) to be served.
  • Ability to function effectively as a member of a multi-disciplinary care team.
  • Excellent interpersonal and communication skills.
  • Computer experience – proficiency with Microsoft Office, Gmail, etc.
  • Demonstrated ability to handle many details and multiple priorities.  Must show an ability to organize, process, and verify details.
