“Lead Clinician (CSS)”
Renders comprehensive health care within the limitations of professional licensing to patients of the Los Angeles Christian Health Centers. Assesses needs and prescribes treatment. Supervises other providers (Advanced Practice Providers)administratively at their primary clinical site or designated sites. Helps lead the clinical and operational activities of outreach sites at LACHC. ESSENTIAL DUTIES, RESPONSIBILITIES, AND PRIVILEGES include:
Supervise other providers (Advanced Practice Providers) employed by LACHC at designated clinical sites (agreed upon with the Chief Medical Officer and Chief Innovations Officer) in an administrative/ Human Resources capacity, conducting performance reviews in a timely manner and disciplinary processes as needed.
This includes thoroughly reviewing at least 10 charts of new medical providers under their supervision, to confirm clinical competence and documentation according to LACHC standards and protocols, and reporting findings to the Chief Medical Officer and/or Chief Innovations Officer.
Provide administrative and clinical guidance to providers under their supervision, responding promptly to requests for help, and helping providers obtain available resources to do their jobs effectively.
Be a leader in active and regular communication with other department leaders, such as the clinic manager, Medical Assistant Supervisor, nurse, and pharmacy director.
Provide clinical leadership at the clinical site or group of similar sites, helping to lead site meetings, serving as a point person for other providers at those sites, and responding to urgent operational and clinical needs at the sites.
Work with the clinic manager, Chief Medical Officer, Chief Innovations Officer and Executive Assistant to the CMO to manage the provider schedule at the sites and respond to changes and call-outs.
Help coordinate clinical and learning activities at the designated clinical site, such as mock codes, journal club meetings, and site- specific meetings.
Serve as a champion in our organization for continuous quality improvement in both clinical and operational measures, working with other clinical staff and the Continuous Quality Improvement team to champion the effective use of care teams through stepwise improvements, including PDSA (plan- do-study-act) measures, participating actively in provider peer reviews, and evaluating clinical performance measures.
Work closely with the Clinical Supervisors of all providers under their administrative supervision, to facilitate effective performance monitoring and provide the most support so that providers can meet their clinical and operational goals and effectively serve according to the mission of LACHC.
As able, actively participate in opportunities to learn and develop their supervisory, managerial, and leadership skills.
Perform evaluation and treatment of patients in the outpatient setting at Los Angeles Christian Health Centers sites. Assess medical needs and compile care plan, addressing acute needs, chronic conditions, and preventive health maintenance. Furnish/transmit appropriate medications and treatments. Perform in-office procedures according to approved privileges such as incision and drainage, arthrocentesis, joint injection, nail removal, skin biopsy, and cryotherapy.
Keep accurate, concise, and thorough documentation of all medical history, exam findings, assessment, treatment/interventions, and education; complete documentation and lock progress notes in a timely manner.
Review and electronically sign off on all laboratory results, diagnostic results, and outside consult notes in a timely manner, ensuring patient is contacted for follow up if clinically appropriate.
Initiate and follow through on specialist referrals and e-consults (electronic consults), communicating with specialist providers as necessary, in the provision of whole- person care.
Serve as the primary care provider for a panel of patients, working with a care team and other staff at LACHC to serve as a patient- centered medical home for our patients.
Actively participate in spiritual care for patients and staff, which may include prayer and referral to community resources.
Provide compassionate care for patients who, depending on the clinic site, are predominately homeless, may suffer from mental illness, and often lack resources to care well for themselves.
Meet productivity standards for outpatient primary care at LACHC, including an expectation of 17-18 patients seen per usual 8 hour day..
Conjointly supervise medical assistants
Participate in LACHC meetings including morning “team huddles”, Provider Meetings, All-Staff Meetings, and supervisory training meetings.
Periodically serve “on call” for the clinic, answering urgent phone calls from patients or other agencies in the off-hours for one week approximately every 10-15 weeks.
Network with other health care agencies and county facilities.
Assist in provision of health screenings (including TB, diabetes, hypertension) at outreach events.
Participate in CQI activities.
Other duties may be assigned.

Bilingual Spanish preferred.
Licensed to practice medicine in California – Advanced Practice Provider: PA or NP. Prior experience in a community clinic is preferred. A copy of California License and DEA license are required. CPR Certification is also required.
